New issue of Safer Radiotherapy newsletter published
The latest issue of Public Health England’s (PHE) Safer Radiotherapy newsletter has been published with key messages and trends analysed from radiotherapy error (RTE) data between December 2018 and March 2019.
Issue 28 for May 2019 contains an IR(ME)R guidance update, information surrounding the uptake of the safety barrier taxonomy and outlines the changes to national patient safety incident reports.
Within the RTE data analysis, the most frequently reported RTE process code in the near miss (level 4) classification was ‘use of on-set imaging’ with 59 reports (7.8%), the document states.
There are also useful publication links, dates for the diary and the risk assessment survey results, which indicated a variation in the use of risk assessments across the UK.
The popular guest editorial feature for this issue comes from Suzanne B Evans, associate professor in the department of therapeutic radiology at Yale University School of Medicine.
Suzanne discusses the importance of encouraging physician participation in incident learning systems.
